Our Leadership
Hometown Home Health was launched in 2020 to address a lack of home health care in the founders' home town of Macclenny, FL. There was a great need, so our founders left the hospitals where they were serving as nurses to start their first entrepreneurial venture.
Several years later, Hometown Home Health has become the preferred agency for many doctors and medical institutions in Northeast Florida. We continue to grow far outside of the original footprint of Macclenny into surrounding communities.
Kristen Gruentzel
Kristen is a co-founder of Hometown Home Health and a seasoned nurse with over 20 years of dedicated experience. She earned her nursing degree from and built a career in the emergency room before transitioning into home health care. Along the way, she found joy and fulfillment as a wife to Jason and mom to three sons. Her family and her faith are central in her life, and she loves to travel with her family to the beach and mountains. Her life and career reflect a dedication to nursing, community service, and family values.
Matti Lee
Matti is a co-founder and VP of Business Operations for Hometown Home Health. She began her nursing journey directly out of high school, completing a Bachelor of Nursing degree. She married her husband, Jeremy—who also works in the business—and grew their family to 5 over the next several years. Matti cherishes the role of "Mama" most of all, and loves the opportunities she has to spend with her family outdoors. Ask her and she'll tell you that she understands God's goodness through both her family and her business.
